You may use this form to report any incident you believe to be a violation of the Wellesley Youth Baseball & Softball Code of conduct.

Upon submitting this form, the information you provide will be directed to the members of the Conduct Review Committee who will promptly review it and determine what action, if any, should be taken.

If you wish to withhold your name, you may do so, but please understand that may hinder the Committee's ability to investigate and/or take action with respect to the incident.
